Health CentreKingsclere Health Centre Opening Hours: Monday to Friday Main Number (day, night, weekends): 01635 296000 When the Health Centre is closed: At all other times when the Health Centre is closed, your call to the Health Centre main telephone number will be transferred automatically to the control room of 'Hantsdoc', our local out of hours general practitioner co-operative. Thee Health Centre is in the centre of the village, immediately behind The Crown public house and opposite St Mary's Church. All parts of the Health Centre are at ground floor level and easily accessible by wheelchair and a disabled toilet is available. Arranging a Consultation The Health Centre receptionists give advice about the Practice and make arrangements for consultations. You may talk to them in person or speak to them on the telephone. You should first descide whether you want to consult with a Doctor or with a Nurse and whether an appointment is necessary. Telephone discussions save time and are often more convenient, both for you and us. Telephone Consultations - in order to avoid unnecessary intreruptions, Doctors and Nurses take only emergency calls during consultations, but will always phone you back later in the day if you leave a message. Sometimes we telephone patients ourselves, to check on progress, to discuss investigation results, or to suggest a visit to the Health Centre. Health Centre Consultations - all consultations are by appointment and are usually ten minutes in length. Sometimes a longer appointment is needed so please tell the receptionist if you think you will need more time. It is helpful if you can give some idea of the reason for your attendance, particularly when booking to see a Nurse. This helps our reception staff to ensure that they allocate an There will often be a few days to wait for a routine consultation, especially if you wish to see a particular person, but if you explain to the receptionist that your need is urgent, you will always be offered an appointment the same day. If for any reason that is not possible, arrangements will be made for you to speak to a Doctor or a Nurse. Home Visits - if you think you may require a home visit, please phone as early in the day as possible. Arrangements will be made for you to speak to the duty doctor, who will either give advice, or arrange for a Doctor or a District Nurse to call, according to the type of help required. Regular visits to those in need of treatment for continuing illnesses are planned at our regular clinical team meetings. Deciding Who to Consult Traditionally patients have consulted their Doctor whenever they have any problem related to their health. We believe that team care is better care, so our patients are looked after by a team of professionals, each with their own skills, training and responsibilities. You can choose to consult with any member of the team, subject to availability and the urgency of the problem. Our reception staff are always pleased to advise whether a Doctor or Nurse, or perhaps one of the managers is best able to help with any particular problem. You can expect to be referred between different team members whenever this is appropriate, but usually it helps continuity of care to consult with the same Doctor or Nurse for the duration of any illness. Medication and Repeat Prescriptions Long Term Medication - patients requiring regular drug treatment may obtain repeat prescriptions without consultation, once this has been authorized by one of the Doctors. In most cases, each individual prescription is for one month's supply of medication but the authorization for the supply of the repeats may last for up to one year. All prescriptions are issued through the practice computer system, with which we carefully monitor on our prescribing. The Doctors are alerted when an authorization expires, or when medication is requested more or less frequently than planned. The Health Centre Dispensary - the Health Centre has it's own dispensary, which is run by the Practice. The Dispensary staff handle all enquiries related to medication and deal with requests for repeat prescriptions. Medications are dispensed for those patients who live more than a mile from the centre of the Requests for Repeat Prescriptions - all telephone requests for repeat prescriptions should be made on the direct line to the Health Centre Dispensary, where our staff have immediate access to medication records on computer. Prescriptions or medications will normally be available within 48 hours of the request being made, but it is best to allow a few days if possible. It always takes longer to process requests for medications that are not authorised for repeat prescriptions or for which the authorisation has been exceeded and there will sometimes be a delay in obtaining more specialised drugs. Prevention is Better than Cure We believe that it is sensible to prevent illness whenever possible and we try to emphasise the importance of a healthy lifestyle. A selection of health education literature is always on display in the waiting area and we have a small library of books on health related topics. Coronary Heart Disease Prevention - heart disease is a major cause of serious ill health and premature death in our society. Our Practice Nurses are available to advise on such matters as the importance of exercise, the dangers of smoking and high blood pressure and the relationship between diet, blood fats and heart disease. Our Health Visitor is involved in helping those who have already had heart attacks to avoid further problems. Family Planning - a full family planning service including 'morning after' emergency contraception, coil and cap fitting and preconception councelling, is offered by both the Practice Nurses and by the Doctors. We can also give advise on sexually transmitted diseases and psychosexual problems. This service is totally confidential and is available in both men and women of all ages in the course of routine consulting sessions. If you need to be seen urgently, it is important to explain this when you arrange your appointment. Women's Health Screening - every woman who needs a cervical smear test, either for clinical reasons or simply as a routine screening procedure, will be notified in writing and invited to make an appointment to see one of our Practice Nurses. Invitation letters or appointments to attend the Immunisations - we offer all the recommended immunisations to children, by invitation, and to adults and those travelling abroad, on request. Mothers and Babies Maternity Care - the care of patients expecting babies is shared between the Doctors and the Midwife (or Midwives) attatched to the Practice. Regular anti-natal and post-natal clinics are held at the Health Centre and all expectant mothers and their partners are invited to attend classes in parentcraft and preparation for labour. Most women choose to deliver in the maternity unit at the Health Visiting for Children - a full time Health Visitor is attatched to our Practice and she spends most of her time supporting and advising mothers of newborn babies and young children. She has regular contacts with local playgroups amd works closely with the school nursing service, community paediatric nurses and with children's social workers. Child Health Clinics - every Thursday afternoon from Baby Changing and Breast Feeding - baby changing facilities are provided in the disabled toilet and we are always happy to accomodate mothers wishing to breast feed. Child Management Meetings - members of the Practice team hold regular meetings with other professionals concerned with the welfare of children, including Community Paediatric Nurses, child Paychologists, children's Social Workers and Doctors and Nurses from School Health Services. Care of Ongoing Medical Problems Patients with long term and recurrent medical problems need careful monitoring and testing, so that complications can be recognised and dealt with as soon as possible. Those taking certain drugs also need to be checked regularly, to identify any side effects of treatment. For these reasons, we invite such patients to attend for review, at intervals appropriate to their needs. Invitations often take the form of a message at the time of issue of the last authorised repeat prescription, but will sometimes come by post or by telephone. We run special clinics for patients with certain conditions such as high blood pressure, diabetes and asthma. The Doctors or Nurses review other patients in their routine consulting sessions. Practice Nursing - our Practice Nurses work almost exclusively at the Health Centre. They do most of the planned regular checks on patients with chronic conditions and they do many of the investigations ordered by the Doctors. In addition, they offer a full family planning advice service, and take most of the cervical smears. They also undertake a range of practical procedures, such as dressings and ear syringing. District Nursing - the District Nurses attached to the Practice are responsible for visiting and treating patients in their own homes. Many of those they visit are elderly and have long term problems from degenerative diseases or are terminally ill. Others are confined for only short periods of time during an acute illness or after an operation. Investigations and Results In order to save our patients having to travel to Other Services Many other important medical, paramedical and social services are available to our patients locally, some at the Health Centre. Examples are physiotherapy, speech therapy, audiology and psychology, also respite and terminal care and specialist nursing. We keep in close touch with the providers of these services and meet with many of them at our weekly team meetings. We can help our patients to access these services whenever appropriate. In some cases, a referral from a Doctor or Nurse is needed. Services not Provided by the NHS The Practice provides a range of services which are not covered by the NHS, for which fees are payable. Full details are available on request from our reception staff or the Practice Manager. Travel Advice and Immunisations - our Practice Nurses offer advice to all patients travelling abroad, free of charge. We have access to world wide travel information, which is regularly updated by the Foreign and Commonwealth Office and the Hospital for Tropical Diseases. We offer all the necessary immunisations (including vaccination against Yellow Fever). Some are available free of charge, but for others private fees are payable.Medical Examinations Medicals for insurance, driving, seafaring and fitness for adventure sports - special appointments are required. Medical Certificates and Signatures - Medical certificates are often needed for employers, travel agents and private medical insurance companies. A range of application and claim forms also require the signature of a professional to whom the applicant is well known, to confirm personal details or to certify good character. Examples are applications for passports, driving licences and shotgun licences. We will help whenever we can.Sports Medical Clinics Dr Wotherspoon is a specialist in Sports Medicine - he and his expert colleagues are available for consultation at the Hampshire Clinic in Event Medical Services - The Practice specialises in providing emergency medical services at sporting and other public events. Often working alongside ambulance service paramedics and members of the voluntary aid societies. Joining the Practice We are happy to accept on to our registered list of NHS patients anyone who lives within the Practice Area (covers a large rural area, most of which is in North Hampshire but a small part of which is in It is very helpful if all patients register with the Practice as soon as they move into the area. It is much better to find out about the services that we offer when you are feeling fit and healthy, rather than waiting until someone is ill. It also enables us to obtain all relevant previous medical records as soon as possible. On joining the Practice, every NHS patient becomes officially registered with one particular General Practitioner and will be issued with a Medical Card bearing the Doctor's name. Despite this, all our patients may consult any Doctor or Nurse of their choosing, subject of course to availability. New Patient Consultations - all new patients are invited to attend for a new patient check soon after registering with the Practice. This gives an opportunity for you to meet one of the clinicians and for us to explain our services to you. It also helps to ensure we have important information about your medical needs as soon as possible. Those with current or ongoing medical problems and those requiring regular medication should see a Doctor. Others will normally see one of the Practice Nurses. Private Patients - a few patients prefer to consult us privately, rather than register as NHS patients. We do not encourage this, because there are few advantages and the costs are high, but anyone who is interested in discussing this option further is welcome to speak either to one of the Doctors or to our Practice Manager. 'Basics' Accident Doctor Scheme The Doctors in the Practice are members of "Basics" (The British Association for Immediate Care), a charitable organisation dedicated to providing well trained and fully equipped Doctors to undertake emergency work at the scene of road accidents and major accidents. In response to requests from the Royal Berkshire Ambulance Trust and the Hampshire Ambulance Trust, we attend many of the serious road accidents occurring in an area that includes the whole of Our role is to co-ordinate with the medical services at the accident scene and, working closely with the paramedics, to provide urgent medical care and pain relief to those who are injured, particularly those who are trapped for long periods of time in road traffic accidents. We are the only Doctors in the area involved in this work, for which we undertake additional training on a regular basis and have gained specialist qualifications. We receive no payment for our work at the scene of accidents and all the funding for items of medical and communications equipment required for our accident work comes from charitable donations. Contributions are always most welcome, and may be made either to the Practice Trust Fund or "Basics Berkshire" or "Basics Hampshire". They will be used to improve the care available to the victims of trauma and also of course our own patients. We hope you will understand that, from time to time, the need to attend accidents or to deal with other emergencies will disrupt routine appointments at the Health Centre.
